For inside mounted shutters you would measure between the side window jambs casings for the width and from the inside edge of the top jamb casing to the sill floor for the height. The most important factor in measuring is the height. When there is not enough depth for the shutters or you wish the shutters to cover over the entire opening trim work and all you would use an outside mount.
Arched openings need to be measured at the center of the top to the bottom. As each window is not created equal the measurements you take at the top of the window will vary slightly from the one taken along the bottom. Measure all windows separately no matter how similar they look.
Shutter height decisions will be based on this measurement. Record the smallest or tightest inside measurement within 1 16 of an inch for width on the order form.

Use a durable steel tape measure do not use a cloth tape. Accurate measurements are essential for a proper installation. Take your tape measure and get the longest distances for the height and width of the window.
